For patients who require a review of their Blood pressure, we would recommend home monitoring in the first instance, if you have access to a Blood pressure machine. These can be purchased from your local pharmacy. This will then be reviewed by a clinician, and any appropriate action taken.

Diabetic reviews are carried out by our Nursing Team. You will receive an invite from us when you are due to be seen. Should it be necessary, you will also be followed up by a GP following your review.
